Partnering to Ensure Community Well-Being
Our public health systems and organizations not only protect community well-being but also educate and empower populations to lead healthier lives. It is essential that these organizations collect and maintain actionable data to effectively understand the health needs of their communities, document the impacts of their interventions, and maintain and attract funding as public health needs evolve.
EVALCORP has partnered with numerous public health agencies to provide program evaluation, needs assessment, strategic planning and training services. Our consulting services help hospitals and other community-based organizations better understand what supports healthy communities and tailor their interventions accordingly.
Public Health Expertise and Clients
EVALCORP has partnered with client organizations in the public health sector. Our partnerships have included local and large government agencies, foundations, nonprofit hospitals, and healthcare organizations.
Our team is experienced in conducting Community Health Needs Assessments (CHNA) that help both rural and urban health care organizations fulfill their community benefit mission. We have partnered with several nonprofit hospitals to provide structured assessments that measure the health concerns and strengths within a hospital’s surrounding community, including both Community Health Assessments (CHA) and Community Health Improvement Plans (CHIP).
We have had the honor to work with organizations supporting a wide range of populations. For example, we conducted a comprehensive needs assessment to obtain information specific to the health needs of adults 60 years of age and older. Surveys and focus groups were conducted among older adults with diverse needs, providers, and caregivers. Our consultants took the time to ensure data accurately reflected the older adult population so that organizations could tailor programming to best meet their needs. Another notable example includes conducting program evaluation and focus groups designed to translate community voices into strategic recommendations for a women’s health program. We also conducted statewide evaluations of a public health campaign in California designed to help prevent the spread of COVID-19 among marginalized groups who were disproportionately affected by the virus.
EVALCORP maintains a culturally responsive approach to public health evaluation and assessment projects. We are skilled in conducting surveys, focus groups, stakeholder interviews, and other methodologies responsive to the community’s needs and context we are working in. Our consultants maintain expertise in conducting trauma-informed research and understand that cultural competence is central to ensuring that data accurately represents all community members.

Case Study: Multi-year Community Health Needs Assessment (CHNA) and Implementation Strategy for a Nonprofit Health System
Client: National Nonprofit Health System
Service Type: Community Health Needs Assessment, Implementation Strategy
Context: A large nonprofit integrated health system engaged EVALCORP across multiple consecutive CHNA cycles to assess and improve the health of diverse communities across California. As a tax-exempt nonprofit system, the client is required every three years to examine how social determinants of health—including structural racism, poverty, and lack of access to affordable housing, healthy food, and transportation—are shaping health outcomes for underserved and unserved residents, and to develop implementation strategies that direct community benefit investments accordingly.
Approach
Across consecutive cycles, EVALCORP developed Community Engagement Plans for each service area and partnered with Community Health Managers to identify data-collection strategies to reach underserved populations. Each cycle drew on quantitative data from public health and Census sources and on qualitative data gathered through interviews with approximately 50 community health leaders, spanning housing assistance, food distribution, free or low-cost community clinics, and health equity organizations. EVALCORP applied formal, evidence-based prioritization methods to identify and rank health needs consistently and transparently across all service areas nationwide. Following each cycle, our team developed Implementation Strategy Plans using a formalized matrix, tailoring each plan to the local community context while maintaining alignment with the system’s national priorities.
Impacts Achieved
Through several multi-year engagements, EVALCORP supported the client in engaging thousands of diverse community stakeholders and producing Implementation Strategies that provided three-year funding and action roadmaps across all service areas. The health system was able to build on prior findings, deepen community partnerships, and continuously improve the reach and relevance of its community benefit investments. As a result, Community Health Managers gained concrete, data-informed evidence to confidently direct resources toward their communities most in need.
