Needs Assessment In Action

Since 2015, Sourcewise has partnered with EVALCORP to conduct a countywide older adult needs assessments and co-develop Santa Clara County’s Four-Year Area Plan on Aging. Below is the most recent Area Plan on Aging that highlights findings from engaging with older adults across various backgrounds and ethnicities (including Vietnamese, Asian American Indian, Chinese, Black, and LGBTQ+) regarding their needs for access to housing, health and mental health services, food resources, transportation, and environmental issues or concerns.

Community Engagement Planning and Implementation

EVALCORP assists with community engagement planning to help you forge meaningful connections with local leaders and hear about their needs directly.
To support your team, we undertake the following:

  • Interview key stakeholders
  • Conduct surveys
  • Host community forums or community listening sessions
  • Arrange and facilitate focus groups
  • Perform trend analysis with existing data

As part of our community engagement implementation services, EVALCORP will also conduct community engagement and outreach activities on your behalf, based on our data-driven plans.

Data Review

Our team will review existing data sources — including internal program data and publicly available databases — and identify standardized health indicators that most accurately reflect the community’s needs. These indicators allow you to compare your community with similar cities, counties, or states to get a better understanding of where your community stands. They will also assist your program in developing benchmarks and an evaluation plan to assess your efforts in addressing your community’s needs. Finally, the findings from our data review can be used to optimize your data collection methods and fill in the gaps of publicly available data instead of collecting what is already available.

Prioritization Facilitation

Prioritization facilitation is a process that helps your team eliminate any ideas, concepts, or topics that do not align with the scope of your program. After your team members have narrowed their focus, they can leverage their combined talents to prioritize projects and initiatives that will make the largest impact on your community. During our needs assessment process, our team will help you prioritize the community’s needs that are most appropriate for your program and scope.

Learning Summits

During a learning summit, we gather stakeholders and community members to share our findings from the needs assessment. Participants will learn vital insights about your initiative, why it is necessary to the community, and how it has a positive impact on participants — facilitating the evolution and optimization of your program.
